Insurance

I am in-network with Pacific Source Commercial, and bill out-of-network with several other insurances companies. 

No Surprises Act

In compliance with with the No Surprises Act, I will provide all who are uninsured or choosing not to use insurance with a Good Faith Estimate upon request and/or prior to a scheduled appointment. The Good Faith Estimate will give you the expected cost for health care related tasks and/or services, including psychotherapy.

 
When you schedule a medical or mental health service at least 3 business days in advance, you have the right to receive a Good Faith Estimate within 1 business day after scheduling. When you schedule a medical or mental health service at least 10 business days in advance, you have the right to receive a Good Faith Estimate within 3 business days of scheduling. You have the right to ask for a Good Faith Estimate before you schedule an appointment, and the health care provider must provide a Good Faith Estimate within 3 business days.
 
In the event that you receive a bill that is $400 more than your Good Faith Estimate, then you can dispute the bill. It is important that you keep a copy and/or take a photo of your Good Faith Estimate in the event that you need to dispute charges for your medical or mental health services.
